Trial Division – s. B.i.1(a) of Code – forged document – Student knowingly altered or falsified a document or evidence required by the University of Toronto, or uttered, circulated or made use of any such altered or falsified document, namely, a signed University of Toronto Verification of Student Illness or Injury Form, which he submitted in support of his petition for a late withdrawal from a course – ss. 6 and 7 of the Statutory Powers Procedure Act - Rules 9 and 17 of the University Tribunal Rules of Practice and Procedure and the University’s Policy on Official Correspondence with Students - finding of guilt - grade assignment of zero for course - two-year suspension – three-year notation on the Student’s transcript - report to Provost of the decision and sanctions imposed, with the Student’s name withheld.
