IMPORTANT- Canada Post Strike
Canada Post workers are on strike and mail is not being delivered. If you are sending anything to our office, please send by courier or drop it off in person.
We will provide updates as soon as possible.
To request a replacement diploma for one that was lost or destroyed, please complete the following steps in this order for each diploma:
- Clear any outstanding fees or financial holds with your Registrar
- Complete the Replacement Diploma and Delivery form in its entirety with your original signature in ink
- Complete a Statutory Declaration and have it signed and sealed by a Notary Public*, legally swearing that the original diploma has been lost or destroyed
- Complete the online Diploma Replacement Payment form and pay $150.00 CAD
- Include the transaction ID from your payment on your Replacement Diploma and Delivery form
- Return your completed original Replacement Diploma and Delivery form and signed and sealed Statutory Declaration to the Office of Convocation**. If returning in-person, make an appointment.

- Wait approximately 4 weeks after your document has been received for notification that your replacement diploma is being delivered by courier. Please allow 6-8 weeks during peak Convocation seasons (early May to the end of June and early October to the end of November).
*A Notary Public is a public official who can authenticate documents with a notarial seal. Many lawyers are also Notaries. A Notary Public is not equivalent to a Guarantor. Please contact a local law office in your area.
**Electronically sent or signed forms will not be accepted. Your original signature in ink must appear on this form.
